Your pregnancy health record

By now, you have probably already received your Pregnancy Health Record (Boletim de Saúde da Grávida), often known as the "green book". This will be one of the most important documents during your pregnancy and should go with you until the birth of your baby and, in many cases, during the postpartum period as well.

Throughout these months, important information will be recorded about your health, the progress of your pregnancy, the results of blood tests and scans, vaccinations, examinations carried out and other relevant data, so that all the professionals involved in your care can know your medical history.

Always take your record with you to your appointments, scans and, later on, when you go to the maternity ward. Having all the information in one place makes it easier for different healthcare professionals to communicate and contributes to safer, more organised care.

The record can be filled in by different healthcare professionals, such as doctors, specialist maternal and obstetric health nurses, and other professionals involved in supporting your pregnancy.

Take a few minutes to look through your record. Don't be afraid to ask about the meaning of any entries or abbreviations you don't understand. This document is yours too, and knowing the information it contains helps you participate in a more informed way in your pregnancy care.
